This is a birth certificate from 1796 registering the birth of a Martha Juxon. It is a common Quaker practice to record births in this style, with a pre-printed certificate to be filled in and signed by witnesses to the birth.

This image reads: On the Nineteenth Day of the Tenth Month, One Thousand Seven Hundred and Ninety Six was born in Birmingham in the Parish of Birmingham in the County of Warwick unto Nicholas Juxon Button maker of Birmingham and Elizabeth his wife, a Daughter whom they named Martha.
We, who were present at the said Birth, have subscribed our Names as Witnesses thereof.
